Sometimes a little extra support can make a big difference.
Our extended care team can help with healthy eating, becoming more active, managing stress, improving sleep, coping with low mood or anxiety, and living well with a long-term health condition.
These services are practical, supportive and tailored to what matters most to you. You do not need to wait until things feel overwhelming, and you do not need to know exactly what type of support you need before getting in touch.
Appointments are free for eligible enrolled and casual patients.
Talk with your doctor, nurse or reception team, and we will help connect you with the right person.
