Who can enrol?
To enrol with Pihanga Health, you need to live within our local service area and be able to attend the practice in-person when healthcare appointments or assessments are required.
You can only be enrolled with one general practice at a time. If you are currently enrolled with another medical practice, your enrolment will be transferred to Pihanga Health once your application is accepted.

How to enrol
Complete the online enrolment form or collect a form from reception.
Complete all required personal, contact and eligibility information.
Provide the requested identification or eligibility documents.
Sign the enrolment and health-information transfer authorities.
Submit the form and wait for confirmation from Pihanga Health.
Transferring from another practice
Pihanga Health may request your clinical records from your previous general practice after the necessary consent has been provided. This helps the clinical team understand your health history and continue your care safely.
