Health Coach
Would you like some extra support to improve your health, but are not sure where to begin?
A Health Coach works alongside patients and whānau to make healthy changes feel practical and achievable. They can help you understand health advice, decide what matters most to you and create a realistic plan that fits your everyday life.
A Health Coach can support you with:
Managing a long-term health condition
Healthy eating and becoming more active
Taking medicines regularly
Reducing stress
Setting achievable health goals
Finding other health or community services
Health coaching is supportive, encouraging and focused on the changes that are important to you.

Health Improvement Practitioner
You do not need to wait until things become overwhelming before asking for support.
A Health Improvement Practitioner provides practical help for concerns affecting your emotional, mental, physical or social wellbeing. Appointments are relaxed, confidential and focused on what would be most helpful to you right now.
A Health Improvement Practitioner can support you with:
Stress, worry or anxiety
Low mood or feeling overwhelmed
Sleep problems
Grief, loss or major life changes
Relationship or parenting concerns
Chronic pain or long-term health conditions
Alcohol, drug or other addiction concerns
Appointments focus on useful tools and practical steps that you can begin using straight away.

Dietitian
Good nutrition advice should be practical, realistic and suited to you.
A Dietitian provides personalised support to help you make food choices that fit your health needs, lifestyle, culture and budget. Rather than providing a restrictive diet, they can help you find manageable changes that work for you and your whānau.
A Dietitian may be able to help with:
Diabetes or prediabetes
High cholesterol and heart health
Healthy weight management
Digestive concerns
Food allergies or intolerances
Poor appetite
Nutrition during illness or recovery
Creating healthier family eating habits
You can ask questions, discuss any concerns about food and receive clear, evidence-based advice.
Eligible patients can access this service free of charge. Talk with your doctor or nurse about a referral.
Exercise Consultant
Becoming more active does not have to mean joining a gym or starting an intense exercise programme.
An Exercise Consultant helps people find safe, enjoyable and realistic ways to move more. They will work with you at your current level and help you build confidence gradually.
An Exercise Consultant can support you if you:
Are returning to exercise after a break
Have a long-term health condition
Want to improve your strength, balance or mobility
Experience pain or physical limitations
Want to improve your fitness
Need help staying motivated
Are unsure what exercise is safe for you
Together, you can create an activity plan that suits your health, abilities and daily routine.
